DOT 2024 Compliance: What You Need to Know

In 2 previous articles we discussed updates and requirements to DOT physicals based off of diabetes and vision here. We also discussed some updates to speed limits and clearinghouse standards here. As of July we wanted to talk about what else changed in 2024.

Can a driver pass with hearing in one ear? This is possible, especially with a hearing aid. We will dive into DOT Physicals with FMCSA standards and guidelines. 

Here is a copy of the 2024 DOT Medical Examiners Handbook section on hearing:

Forced Whisper Test: The driver must be able to perceive a forced whispered voice in the better ear at a distance of not less than 5 feet, with or without the use of a hearing aid.

Audiometric Test: If tested using an audiometric device, the driver must not have an average hearing loss in the better ear greater than 40 decibels at frequencies of 500 Hz, 1,000 Hz, and 2,000 Hz, with or without a hearing aid. The audiometric device must be calibrated to American National Standard (formerly ASA Standard) Z24.5-1951.

Use of Hearing Aids: If the individual meets the hearing requirement with a hearing aid, the hearing aid must be used while driving. The “Wearing hearing aid” box must be selected on the Medical Examination Report Form, MCSA-875, and the Medical Examiner’s Certificate, Form MCSA-5876.

So what was new about this?

Between 2021 and 2024 this is what changed: Drivers were required to use hearing aids if needed to meet hearing standards, but the documentation requirements were less explicit compared to the updated regulations. In addition the checking of the box is a new regulation to clarify further that this person has passed, but must be wearing their hearing aid. So, don’t forget your hearing aid when coming to the test!

There are also some clarifications around blood pressure and hypertension. Before we dive in do you get nervous when visiting the doctor or lab? This can cause your blood pressure to be elevated and perhaps send you over the limit for passing. Anxiety can possibly raise your blood pressure by up to 10 points! Practice deep breathing, wiggle your toes, stick to water the week before and wear warm testing, especially if you get cold easily. Talk to us about your anxiety before your DOT or Department of Transportation physical. 

The 2024 guidelines place additional emphasis on confirming elevated blood pressure findings with subsequent measurements and the evaluation of side effects from antihypertensive medications, and the procedural aspects and recommendations for certification periods have been clarified, particularly for individuals with Stage 2 and Stage 3 hypertension.
