Why Fit Testing Isn’t Enough Before Employees Wear Respirators

Many employers understand that respirator fit testing is required when employees must wear respiratory protection. However, one critical step is often overlooked: the medical evaluation that must occur before fit testing and respirator use.

A respirator places additional stress on the body. Depending on the type of respirator, employees may experience increased breathing resistance, elevated heart rate, heat stress, and physical strain during use. While most healthy workers tolerate respirators well, some medical conditions can make respirator use difficult or even unsafe.

OSHA’s Respiratory Protection Standard (29 CFR 1910.134) requires employers to provide a medical evaluation before an employee is fit tested or required to wear a respirator. The purpose of the evaluation is to determine whether the employee is medically able to perform their job while wearing respiratory protection.

Medical evaluations may identify conditions such as:

  • Asthma or chronic respiratory disease
  • Cardiovascular conditions
  • Claustrophobia or anxiety disorders
  • Recent surgeries or injuries
  • Other health concerns that could affect respirator use

Without a medical evaluation, an employee could successfully pass a fit test yet still face health risks while working in a respirator. A fit test only verifies that the respirator seals properly to the face. It does not determine whether the employee can safely wear the equipment for extended periods or during physically demanding tasks.

A complete respiratory protection program includes:

  1. Hazard assessment and respirator selection
  2. OSHA-compliant medical evaluation
  3. Respirator fit testing
  4. Employee training
  5. Ongoing program review and maintenance

Employers who focus solely on fit testing may unintentionally leave a significant compliance and safety gap. Medical evaluations help protect employees, reduce liability, and ensure OSHA compliance before workers enter environments requiring respiratory protection.

At MedPhysicals Plus, we help employers throughout Alaska complete respiratory medical evaluations and fit testing services as part of a comprehensive respiratory protection program. Whether your workforce operates in construction, manufacturing, healthcare, oil and gas, transportation, or remote worksites, ensuring employees are medically cleared before fit testing helps create a safer workplace for everyone.

Remember: a respirator that fits is important—but a respirator worn by a medically cleared employee is what truly provides protection.
